Commercial Driver Training Welcome Packet Checklist

Please review the information below for enrollment information.
All tasks must be completed at least 10 days before the class’s start date.

1. Apply to Spoon River College Complete the SRC Admissions Application form. Be sure to select Commercial Driver Training as your Intended Major. If you are already a student, skip this step.
2. Complete the CDT Application Please complete and submit the electronic CDT Application form.
3. Apply for Financial Aid This step is optional, but can be helpful. More information can be found on our Financial Aid webpage. The CDT webpage also has some additional funding sources.

Tuition and Fees: $4,850.00 total, 25% is due at the time of enrollment ($1,212.50) and the remaining 75% is due by first week of class ($3,637.50).

4. Obtain a DOT physical and Drug Screen Visit the Federal Motor Carrier Safety Administration website for additional information on the required DOT physicals and Drug Screening. DOT physicals and drug screenings must be conducted by a licensed “medical examiner” listed in the National Registry of Certified Medical Examiners. Students must receive the Medical Examiners Certificate form from their licensed medical examiner and the results from their drug screening to bring with them to their appointment with the CDT Program Director in order to be approved to enroll in the program (step 5).
5. Make an appointment with the CDT director, Michele Johnson To complete your enrollment, you must make an appointment with the CDT Director

You will need to bring the following items to this meeting to be enrolled in the CDT course:

  • Certified Birth Certificate (must have a raised or stamped seal)
  • Driver’s License
  • SRC Student ID number
  • Copy of your Medical Examiners Certificate, from your DOT physical
  • Copy of your Drug Screening Results

You will not be enrolled if you fail to bring any of these items to your enrollment appointment.
